A landmark 2013 paper in Cell identified the hallmarks of biological aging. A 2023 update expanded them to twelve. These are molecular processes rather than cosmetic signals, and many of them respond to the same inputs the classical tradition has prescribed for millennia: what you eat, when you move, how you manage stress, and whether your daily rhythm aligns with the biological clock.

Genomic instability, telomere attrition, epigenetic alterations, loss of proteostasis, disabled macroautophagy, deregulated nutrient-sensing, mitochondrial dysfunction, cellular senescence, stem cell exhaustion, altered intercellular communication, chronic inflammation, and dysbiosis. A systematic review of 20 studies (2,995 participants, mean age 50, 77% women) found physical activity and diet interventions produced a small but significant increase in telomere length.
